This report is to be completed when an incident occurs on Timberlake Church property.If an employee is injured in the course of employment, follow the instructions as outlined in the Worker’s Comp section instead of completing the incident report.
Worker's Comp
When an employee is injured: Call Beth Francis or Ruth Boyd as soon as you become aware of an employee being injured in the course of employment.Print & complete all sections of the Virginia First Report of Injury form except the submitter section at bottom of the form.Email or give the form to Beth Francis or Ruth Boyd ASAP after the incident occurs. Have the employee take the Virginia First Report of Injury form with them to receive care at one of the approved care facilities as listed, or call 911 if necessary.Beth or Ruth will complete the submitter section of the form and email it to the insurance company to initiate the claim.
